Week 4 Assignment: Process Improvement Plan Paper

Objective: Create a process improvement plan.

Instructions

Implement a process improvement plan for patients in a hospital.

Choose from one of the following three scenarios:

• Patients receiving follow-up medication after a hospital visit
• The intake process for the children’s clinic
• Care coordination for a patient under the care of four or more providers

Include a completed table, like the one below, for your process improvement. Hint: See Gap Analysis Definition and Figure 6.5 in your text.

Compose a two- to three-page paper in APA Style. Your paper should include a cover sheet and reference list. Submit the completed table with your written paper.

Include an introduction to your paper and conclude with a summary evaluating the effectiveness of the process improvement plan.

You must cite and reference at least two sources. You may use your textbook as a source.
